- Individuals who are members of the Uintah and Ouray Reservation and their descendants.

- Photo ID — Driver's license or state-issued ID and tribal identification; Certification of Indian blood; Court order (if applicable); Birth Certificate — Descendents of enrolled tribal member must also bring birth certificate.
