Who can use this
- Eligible donors are women in good overall health, non-smokers, and not regularly taking medications. They will receive a detailed screening form to complete and return.
Documents you may need
- Potential donors will be sent a comprehensive screening form to fill out and send back. Donor consent form and your doctor’s signature for the medical release form.
