This program helps individuals with disabilities who are currently residing in nursing homes or similar institutions to move into community-based living arrangements. It also offers assistance with utilizing technology and applying for Section 8 housing.
Who can use this
- Applicants must have a documented disability and have lived in an institution for a minimum of 3 months.
