This federally funded health insurance initiative caters to individuals aged 65 and older, along with younger individuals with disabilities who have either received or been deemed eligible for Social Security Disability benefits for a designated period.

Who can use this
- - Citizens of the United States who are 65 years or older - Citizens under 65 who have been on Social Security disability benefits for a minimum of 24 months
Documents you may need
- Social Security Card — Social Security Number; Proof of Insurance — Start and end dates of any current group health plan and end dates for any group health plans after age 65
