This initiative supports low-income women, infants, and children under the age of five by providing nutrition education, vouchers for healthy foods, breastfeeding assistance, immunization record assessments, and connections to additional helpful organizations.
Who can use this
- Eligible participants include females who are pregnant or breastfeeding, new mothers, or children under five years old who meet specific income criteria.
Documents you may need
- Proof of Address — Proof of residency (address) record for each infant/child.; Income Verification — Proof of income record for each infant/child.; Photo ID
