Intake Procedure: Applications can be submitted online at: https://swuhealth.gov/wic/

- Eligible participants include pregnant or breastfeeding women, recent mothers, or children under five who meet specific income criteria.

- Proof of Address — Proof of residency (address); Income Verification; Photo ID; Immunization record for each infant/child.
