About this resource
Provides two home visitation services: Targeted Case Management (TCM) and Prenatal to 5. These visits consist of assessments of developmental milestones, support for parents and caregivers, and connections to medical, social, educational, and various other support services when necessary.
Who can use this
- A referral is necessary for the Prenatal to 5 program, while Targeted Case Management is exclusively available to Medicaid recipients.
