Provides a daytime healthcare service aimed at adults facing difficulties in areas such as cognitive, emotional, physical health, and social interactions.
Intake Procedure: Reach out to the agency to schedule an intake appointment; a referral is necessary for those on Medicaid.
Fees: Accepts Apple Health (Medicaid), Long Term Care Insurance, Private Pay, and has a sliding fee scale. Medicaid is only accepted if the patient is referred by a case manager.
Who can use this
- Participants must be 18 years or older, require supervision or monitoring of a health condition, and need ongoing support.
