About this resource
This initiative fosters collaboration between medical and behavioral health professionals to work together with patients. It evaluates immediate needs and objectives, providing recommendations for strategies that promote a more balanced lifestyle. Participation requires that individuals are receiving services from the family practice clinic.
Intake Procedure: Individuals can enroll in services at the family practice clinic, where behavioral health clinicians are available to see patients, frequently on the same day, eliminating the need for separate appointment scheduling.
Fees: The program accepts WA Apple Health (Medicaid), Medicare, private insurance, private pay, and provides a sliding scale fee option.
