This program is designed for very low income individuals and families who have experienced chronic homelessness and have disabilities related to physical health, mental health, or substance abuse.
Intake Process: Contact via phone or email, and appointments are necessary.
Costs: Fees depend on placement, with rents for permanent supportive housing capped at 30% of income.
