This initiative delivers community-oriented outreach, assessment, and service connections, including long-term intensive case management and recovery support services when appropriate. It caters to both adults and youth facing substance use disorders.

Intake Procedure: Engage with the local program. Generally, clients are referred by law enforcement, community organizations, and the judicial system; however, all types of referrals are welcome.
