Intake Procedure: To obtain information or schedule an appointment, please call. It's important to confirm your appointment by noon the day prior; otherwise, it will be rescheduled. For appointments on Monday, confirmation is required by Thursday at noon.
Fees: There are no charges.
Who can use this
- Eligible individuals are those under 65 years old without medical insurance (not qualifying for Apple Health) and whose income is below three times the Federal Poverty Guidelines, which are subject to change: $3,765 monthly for a single-person household, $5,110 for two, $6,455 for three, $7,800 for four, and $9,145 for five.
