About this resource
This program assists older adults and individuals with disabilities in accessing necessary services by evaluating their needs and coordinating with service providers. It also provides additional support for Medicaid recipients who might otherwise require placement in a nursing home.

Who can use this
- Individuals aged 60 and above, adults with disabilities aged 18 and above, and family caregivers residing in Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, or Asotin counties.
