Intake Procedure: A referral from a physician is required.
Fees: The program accepts Medicaid, Medicare, private insurance, and out-of-pocket payments.
Who can use this
- Requires a physician's certification of terminal illness and a life expectancy of six months or less, confirmed by the Hospice Medical Director.
