This program assists individuals in navigating and accessing community resources to support their independent living, specifically targeting non-English-speaking Chinese seniors, and requires a referral from CLC.
Intake Procedure: A Community Living Connections specialist is responsible for completing and sending the necessary form to a CLC lead via email.
Fees: There are no fees associated with this service.
Who can use this
- Eligible participants include Chinese-speaking adults aged 60 and above or those aged 18 and older with disabilities who need multiple services or related activities but cannot obtain them independently or have no family or friends to assist. Individuals residing in assisted living facilities, adult family homes, or skilled nursing facilities are not eligible, though those in independent senior living communities may qualify. Clients receiving state long-term care services, such as COPES, are also excluded. The focus is on those needing help with personal care and daily living tasks, particularly those who are homebound or isolated due to health issues, as well as frail individuals requiring cognitive support. Additionally, applicants must not be receiving case management from Home and Community Services or a housing authority.
