About this resource
This class series assists individuals in adopting healthy habits to prevent type 2 diabetes, aimed at those 18 and older who are pre-diabetic or at high risk.
Intake Procedure: You can register online for an information session, attend in person, or make a phone call to get started.
Fees: The full cost of the program is usually covered by Traditional Medicare and certain Medicare Advantage plans. For individuals without Medicare coverage, a sliding fee scale is available.
Who can use this
- Participants must be 18 or older with a BMI over 25 (over 23 for Asian individuals), or at high risk for Type 2 Diabetes based on specific medical criteria; Y membership is not necessary.
