About this resource

Delivers extensive primary care, case management, and home care services for individuals aged 55 and above, along with a day health center and the supply of durable medical equipment.

Intake Procedure: Contact the intake representative to undergo a phone screening for eligibility. A center tour or home visit can be arranged to further discuss the program. Should the applicant decide to enroll, the intake representative will guide them through the necessary paperwork.

Fees: Enrollment requires being on WA Apple Health (Medicaid), and individuals may also have Medicare/Medicaid.

Who can use this

Individuals must be 55 years or older, reside in most of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily serving those on WA Apple Health (Medicaid) or Medicare/Medicaid.
