About this resource

Provides extensive primary care, case management, and home care services for individuals aged 55 and older, including a day health center and access to durable medical equipment.

Intake Process: Contact the intake representative to undergo a phone screening for eligibility. A visit to the center or a home consultation can be arranged to further discuss the program. If the individual decides to enroll, the intake representative will help with the necessary paperwork.

Fees: Eligibility requires enrollment in WA Apple Health (Medicaid), and individuals may also have Medicare/Medicaid.

Who can use this

Participants must be 55 years or older, reside in most areas of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily serving those with WA Apple Health (Medicaid) or Medicare/Medicaid.
