About this resource
This class series is aimed at helping individuals adopt healthy habits to prevent type 2 diabetes, specifically targeting those aged 18 and above who have pre-diabetes or are at high risk.
Intake Procedure: You can register online for an informational session, attend in person, or make a phone call.
Fees: The full cost of the program is generally covered by Traditional Medicare and some Medicare Advantage plans. For individuals not covered by Medicare, a sliding fee scale is available.
Who can use this
- Participants must be 18 years or older, have a BMI over 25 (or over 23 for Asian individuals), or be at high risk for Type 2 Diabetes based on specific medical criteria, with no requirement for Y membership.
