This program offers culturally sensitive and comprehensive care to older adults and ADS Medicaid long-term care clients, including counseling, home visits, screenings, and the development of treatment plans.

- Eligible individuals include older adults aged 60 and above, as well as ADS Medicaid Title XIX Case Management Core Clients aged 18 and older, who have been referred by ADS case managers.
