About this resource
This program provides continuous support services and case management for adults with developmental disabilities. A referral from a Developmental Disabilities Administration Case Manager is necessary to participate.
Intake Procedure: Individuals must be enrolled with the Developmental Disabilities Administration and obtain a referral from a DDA case manager. Once the referral is secured, a phone call is required.
Fees: There are no fees associated with this service.

- A referral from a Developmental Disabilities Administration Case Manager is required, and applicants can have diverse needs ranging from severe intellectual disabilities needing constant supervision to those requiring only financial or medical support.
