About this resource
This program assists individuals in navigating and utilizing community resources to maintain their independence, primarily targeting non-English-speaking Chinese seniors, and requires a referral from CLC.
Intake Process: A Community Living Connections specialist is responsible for completing and sending the necessary form to a CLC lead via email.
Costs: There are no fees associated with this service.
Who can use this
- Eligible participants include Chinese-speaking individuals aged 60 and above or those aged 18 and older with disabilities who need multiple services and cannot obtain them independently, lacking support from family or friends. Those in assisted living or skilled nursing facilities are not eligible, but individuals in independent senior living communities can apply. Clients receiving state long-term care services, such as COPES, are not eligible. The focus is on individuals requiring help with personal care and daily living activities, who are homebound or physically isolated due to health issues, and those who are frail and need cognitive assistance. Applicants must not be currently receiving case management from Home and Community Services or a housing authority.
