Desc's 216 Clinical Services

This service delivers mental health case management, psychiatric care, medication supervision, limited therapy, and protective payeeships to individuals who are homeless or have a history of homelessness and are chronically mentally ill. Self-referrals are accepted.

Intake Process: Interested individuals should call to connect with the SAGE intake specialist. Self-referrals are permitted, and professionals can also reach out directly to refer clients. Same-day service screenings may be available, and intake appointments will be scheduled within ten days following the service request.

Costs: The program is mainly funded through WA Apple Health (Medicaid), but clients without Medicaid can pay for services out-of-pocket. Some individuals may be eligible for alternative funding sources.
