About this resource
This program offers tailored services to help nursing-home eligible individuals maintain independence at home or within the community, including medical care, adult day care, and in-home assistance.
Intake Procedure: To begin, call to set up an appointment with an enrollment specialist. An interdisciplinary team will evaluate the patient's medical history to assess the suitability of PACE.
Fees: The program accepts Medicare, Medicaid, and private payments.
Who can use this
- Individuals must be 55 or older, require nursing home level care as per Washington state guidelines, reside in the PACE service area at enrollment, be capable of living safely in the community with PACE support, and be Medicaid-eligible or willing to pay privately.
