About this resource
This initiative provides free home evaluations and suggestions aimed at supporting older adults who are at significant risk of falling. Eligibility requires that individuals have either called 911 due to a fall or been assessed by a healthcare professional as being at high risk for falls.
Intake Process: Interested individuals can call or email for additional details or to arrange an appointment. Healthcare providers can refer patients to the program if they meet the necessary criteria. Adults are also welcome to self-refer if they qualify. King County EMS reviews 911 calls to pinpoint fall-related incidents involving older adults who needed EMS assistance due to a fall. Contact from the program's Physical Therapist or Health Educator may take several days to over a week.
Costs: There are no fees associated with this program.
Who can use this
- Adults residing in King County, outside of Seattle, who meet the following conditions: •At least 50 years old AND •Living independently, not in nursing homes, assisted living, adult family homes, retirement communities, or hospice AND •Must be able to walk AND •Have experienced a fall in the last 6 months and called 911.
