About this resource
This initiative offers nutritious, home-cooked meals delivered to participants' homes within a day of their hospital discharge. Additionally, it includes wellness check phone calls for a period of 30 days starting from the first meal delivery. Eligibility requires participants to reside in South King County.
Intake Procedure: Interested individuals should call for further details. Availability of the program is contingent upon funding.
Fees: There are no costs associated with this program.
Who can use this
- Participants must be referred by a medical provider within two weeks of discharge, have an income at or below 200% of the Federal Poverty Level, and reside in King County.
