Offers assessments, services, and assistance to families with infants and toddlers who may have developmental delays or disabilities.
Intake Process: Contact the Referral line. Enrollment is necessary at least 45 days prior to the child's third birthday. A multi-disciplinary team evaluation is conducted before the child joins the Early Intervention program. Following this evaluation, eligibility for services is determined, and an Individual Family Service Plan is developed.
Costs: All insurance plans are accepted.
Who can use this
- Families with children from birth until their 3rd birthday are eligible. Home-based services are based on ZIP code. A child must exhibit a 25% delay in one or more developmental areas, have a medical diagnosis, or receive an informed clinical opinion.
