About this resource
This program delivers extensive primary care, case management, and home care services for individuals aged 55 and above, featuring a day health center and the supply of durable medical equipment.
Intake Procedure: Prospective clients should contact the intake representative to undergo a phone screening for eligibility. A tour of the facility or a home visit can be arranged to further discuss the program. If the individual chooses to enroll, the intake representative will guide them through the necessary paperwork.
Fees: Participants must be enrolled in WA Apple Health (Medicaid) and may also have Medicare/Medicaid.
Who can use this
- Eligible clients are those aged 55 or older, residing in most areas of King or Snohomish County, and certified as functionally eligible for PACE (or COPES) by DSHS, primarily targeting individuals with WA Apple Health (Medicaid) or Medicare/Medicaid.
