About this resource
Provides extensive primary care, case management, and home care services for individuals aged 55 and older, including a day health center and durable medical equipment.
Intake Process: Contact the intake representative for a phone screening to determine eligibility. A center tour or home visit can be arranged to discuss the program in more detail. Should the applicant decide to enroll, the intake representative will guide them through the necessary paperwork.
Costs: Enrollment requires being on WA Apple Health (Medicaid), and individuals may also have Medicare/Medicaid.
Who can use this
- Applicants must be 55 years or older, reside in most areas of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily targeting those on WA Apple Health (Medicaid) or Medicare/Medicaid.
