About this resource
Provides extensive primary care, case management, and home care services for individuals aged 55 and older, including a day health center and durable medical equipment.
Intake Procedure: Contact the intake representative to undergo a phone screening for eligibility. A tour of the facility or a home visit can be arranged to further discuss the program. If the individual decides to enroll, the intake representative will help with the necessary paperwork.
Fees: Enrollment requires being on WA Apple Health (Medicaid) and may also include Medicare/Medicaid.
Who can use this
- Participants must be 55 years or older, reside in most of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily serving those with WA Apple Health (Medicaid) or Medicare/Medicaid.
