About this resource

This program provides four tailored sessions with a community health worker focused on asthma management, along with a home assessment and supplies to help reduce asthma triggers for children aged 6-12 in King County who have poorly controlled asthma.

Fees: There are no charges.

Who can use this

  • Must be a resident of King County, consent to home, video, or phone visits, have poorly controlled, moderate, or persistent asthma, and be between the ages of 6-12.
