- Families with children aged birth to 18 who have complex health needs or have recently received a diagnosis of a complex medical condition and require assistance with: Finding or coordinating medical providers and a medical home, accessing local resources or services for the child, obtaining financial aid for health-related needs, and understanding or coping with the child's health condition.
