Intake Procedure: Referrals are accepted exclusively from Community Living Connections.

Who can use this
- Adults aged 62 and above or those with disabilities in King County seeking long-term care services, with a monthly income below 800% of the Federal Poverty Level and less than $2000 in non-excluded resources.
