About this resource
This service offers one-on-one meetings with older adults who require extra support during transitions or changes in their lives, such as adapting to a new health issue, relocating, or experiencing shifts in their social circles. The program assists in establishing goals and linking individuals to available resources.
Intake Procedure: To refer someone, please call. Referrals are accepted from Community Living Connections specialists as well as individuals calling on behalf of the client. It is essential that CLC specialists provide clients the choice to call themselves and refrain from contacting on behalf of a third party. They may only represent a client if they have received explicit consent. If there is no response, leave a voicemail containing comprehensive information about the client, including: full name, phone number, address, date of birth, reasons for needing Options Counseling, and a description of the required services. Third parties can also make referrals by leaving the client's details via phone. Sound Generations will reach out to the client within a week, but can respond sooner for urgent cases.
Fees: There are no charges for this service.
Who can use this
- Eligible participants include individuals aged 60 and older, as well as those aged 18 and above with a disability, who need assistance in making care decisions while being able to access services independently, and who reside in King County without currently receiving case management from Home and Community Services or a housing authority.
