About this resource
This initiative assists Medicaid recipients facing behavioral or physical health challenges by providing supportive services that are person-centered and non-tangible, aimed at helping them find and retain employment.
Intake Procedure: Interested individuals can call or visit the website to complete a referral form. Referrals may also come from a healthcare provider or health plan.
Fees: There are no associated costs.
Who can use this
- Individuals aged 16 and older who are Medicaid recipients with complex needs, including those whose age, disability, health issues, or substance use treatment complicate their ability to maintain employment, may qualify; participation in the Housing and Essential Needs (HEN) program is also considered, but does not ensure eligibility.
