About this resource
This initiative delivers person-centered, non-tangible supportive services to Medicaid recipients dealing with behavioral or physical health challenges, assisting them in connecting to and sustaining housing.
Intake Procedure: Interested individuals should either call or visit the website to complete the referral form. If they are already working with a social service provider for case management, that case manager can also submit a referral on their behalf.
Fees: There are no associated costs.
Who can use this
- Individuals aged 18 and above who are Medicaid recipients with behavioral or physical health challenges.
