About this resource
Case managers assist youth aged 13-24 in navigating life transitions by offering assessments, referrals, and life skills counseling. They also support these young individuals in finding housing, enrolling in educational institutions, accessing medical services, and linking with other community resources.
Intake Procedure: Clients can come in person or be referred by others.
Fees: No charges apply.
Who can use this
- Youth aged 13 to 24 who are homeless or at risk.
