About this resource
This service offers compassionate end-of-life care for terminally ill individuals, including bereavement support for 15 months and specialized care for veterans in King, Pierce, and South Snohomish counties.
Intake Procedure: To initiate the process, call and request to speak with the Intake Department for intake or referral. Referrals can come from physicians, patients, family members, or health and social services personnel. Following the initial home visit, a patient care plan will be developed. A physician's order is required to start services.
Fees: Accepts Medicare, WA Apple Health (Medicaid), and other third-party payers. Care will not be denied due to an inability to pay.
Who can use this
- Individuals of all ages with a prognosis of six months or less, as determined by their physician, must seek comfort care and reside in King, Pierce, or South Snohomish County.
