About this resource
This program assists older adults and individuals with disabilities in accessing services by evaluating their needs and coordinating with service providers. It also provides extra support for clients on Medicaid who might otherwise qualify for nursing home placement.

Who can use this
- Individuals aged 60 and above, adults with disabilities aged 18 and older, and family caregivers residing in Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, or Asotin counties.
