This program offers case management services led by a Geriatric Mental Health Specialist to help clients connect with community resources for independent living.
Intake Process: To apply for services, call and give the receptionist essential details such as payment information and the purpose of your call. A face-to-face appointment will then be arranged.
Costs: The fees vary based on the services needed and insurance coverage. WA Apple Health (Medicaid) and private insurance are accepted, and a sliding scale fee may be available based on individual financial situations.
Who can use this
- Eligible individuals are those aged 60 and above who are experiencing mental illness.
