Case Managers evaluate the temporary and permanent needs of older and disabled adults receiving Medicaid in-home personal care services to effectively coordinate these services, promoting their independence.

Fees: There are no fees.
Who can use this
- Adults aged 60 and above, and/or those with disabilities, who pass a needs-based and financial eligibility review conducted by the DSHS Home & Community Services office in Lewis, Mason, and Thurston Counties.
