Offers housing navigation assistance to individuals facing significant health and social challenges through Foundational Community Supports (FCS).

Who can use this
- FCS (Flexible Community Supports) is designed for individuals enrolled in Apple Health (Washington State Medicaid) with significant health-related needs as determined by a qualifying assessment. To qualify, participants must meet certain age criteria and exhibit at least one high-risk factor, such as chronic homelessness, complex behavioral health issues, co-occurring mental health and substance use disorders, or a disability requiring long-term care. This Medicaid benefit focuses on those with the greatest need for housing or employment stability, rather than all Medicaid recipients. A formal referral process is necessary, along with approval from the state’s FCS administrator, currently Wellpoint.
