About this resource
This service offers a complimentary intake to assist in identifying appropriate care options. It includes a team of nurses, care managers, and Placement Navigators dedicated to helping families make informed choices regarding long-term care, home care, and additional needs. There is also an instructional page available for Medicaid.
Intake Procedure: To get started, you can call or submit an inquiry online. If you reach voicemail, please ensure you leave your name and phone number clearly. Additionally, individuals are welcome to visit in person from 9am to 5pm on weekdays.
Fees: The services provided by Placement Navigators for placement or home care are offered at no charge. Each client will receive a full disclosure statement.
