This initiative provides companionship, nutritional support, social engagement, assistance with correspondence, transportation to medical appointments and essential shopping, along with information and advocacy for seniors or disabled adults who are socially isolated.
Intake Procedure: Fill out the online referral form or contact the intake hotline.
