Intake Process: Complete the online application via the "contact us" form and submit it.
Fees: Accepts major insurance plans and private payments. Insurance providers include Washington Apple Health (Medicaid), Medicare (Part B), Tricare, Regence, Blue Cross Blue Shield, Cigna, Premera, Kaiser Permanente, Molina, United Healthcare, Lifewise, KPS, First Choice, Community Health Plan of Washington, Uniform Medical, and various Employee Assistance Programs.
