This program offers mobile assertive community treatment through a multi-disciplinary team, focusing on serving individuals in their own residences.
Intake Process: Requires a referral from a community partner.
Costs: Covered by WA Apple Health (Medicaid).
Who can use this
- Adults aged 18 and above who have experienced multiple behavioral health hospitalizations, numerous incarcerations, unsuccessful housing placements, and chronic homelessness, as well as those with co-occurring substance use disorders.
