This program assists adults with difficult behaviors in moving from a hospital setting to a long-term care facility.
Intake Procedure: Please make a phone call to initiate.
Fees: Covered by WA Apple Health (Medicaid).
Who can use this
- Adults facing serious mental health issues, dementia, mood disorders, or other neurological conditions are eligible, particularly if they have challenging behaviors that hinder hospital discharge or threaten their placement in long-term care.
