This program upholds state laws that protect individuals from discrimination based on their protected status, including those who feel they were unjustly denied a reasonable accommodation related to vaccine mandates.
Intake Procedure: To initiate a complaint, visit our website and select “File a Complaint” on the right. Then, click on Tab 5 – File a complaint online. To submit a complaint online, you will need a Washington Secure Access (SAW) account, with instructions available online. Alternatively, you can download or complete the fillable PDF complaint forms that correspond to your issue and submit them via mail, fax, or email.
Fees: There are no fees associated with filing a complaint.
Who can use this
- Individuals who believe they have faced discrimination based on various factors including race, color, national origin, creed, sex, pregnancy, sexual orientation, gender identity, veteran status, disability, service animal use, HIV or Hepatitis C status, marital status, breastfeeding, age (40+ for employment), families with children (housing), or as a state employee whistleblower.
