Individuals can find order forms on the website, and copies of these records can be requested online. For those experiencing homelessness, birth certificates are provided at no cost.
Intake Procedure: Visit the service counter to fill out a form. Order forms can also be accessed via the website, and requests can be made through phone, mail, or fax.
Fees: The starting fee for each certificate is $25, with extra charges for expedited delivery. Birth certificates are free for individuals who are homeless if the request is made by a service provider.
