Intake Procedure: You can submit a complaint by filling out an online form or by calling. It's important to provide detailed information to assist in resolving your issue. Alternatively, you can print the form, complete it, and mail it to Complaint Intake, PO Box 47857, Olympia WA, 98504-7857.
Fees: There are no fees associated with this process.
